To apply for a Spirit pack for your child/children please read the 2 page instruction sheet listed below. Download and complete the PRC application and order form below.  1 order form per child is required.  If you have 2 children you are applying for you will need to complete 1 application and 2 order forms.  When submitting your application you must submit the application, the order form(s) and  all verifications listed on the checklist below.
